The United Keetoowah Band of Cherokee Indians in Oklahoma Tax Commission will be providing wheelchair ramps for tribal members who qualify.
Applicants must submit an application for the wheelchair assistance program, with all requested attachments to be considered. The attachments are: Written documentation from a physician stating that applicant is permanently disabled/handicapped; proof of home ownership or written permission from landlord; and an estimate from Keetoowah Construction, Inc. (to be provided by Federal Programs Director).
